Demand Draft in the name of ‘the film writer’s association’ for INR 2613/-
Form to be Filled (Please mention ‘Refered by Sankalan’)
For registration with FWA
Concept / Script written with top half of first page blank
Cash / cheque of total amount (calculated @ Rs 10/- for first 5 pages and then @ Re1/- per subsequent page)
Please attach a letter of authority alongwith the above authorising us to do the needful. the letter should mention the list of contents (ie the DD alongwith the name of the bank and number, the cheque and its details, the name of the concept / script and the number of pages for each , and any other details that you feel are necessary)
